Within this program, Youth Referrals between the ages of 15-24 are processed into the Youth Assist Early Intervention Stream and an initial Triage appointment is conducted.
- During the Triage appointment, Clients are informed about their rights of confidentiality and any limitations to this confidentiality, including mandatory reporting.
- A Triage appointment gathers information on the Client including any historical or current diagnoses, Child Safety involvement, exposure to Domestic and Family Violence, experiences of Sexual Assault, any legal interventions and assesses any risks of Suicide Ideation.
- Finally, Clients complete a Readiness for Change Assessment, which is a tool used to evaluate a Client’s motivation, willingness, and ability to engage in support services or make meaningful changes in their life. This tool aids the Youth Assist Program by tailoring interventions to match the Client’s current stage of change.
- The ‘Contemplation Stage’ of Readiness for Change Assessment is the minimum standard for the Youth Assist Program. This stage entails that Client has acknowledged that an issue exists and starts thinking seriously about making a change, however they acknowledge that they might need guidance or help in making these changes.
